How To Make An Appointment With HSR

Active Duty Service Members

  • Step 1: Make appointment with or contact your Primary Care Provider. If you don’t have one assigned, call your command’s medical facility or Tricare for assignment.
  • Step 2: Request authorization for mental health services from your military provider specifically from HSR. You will need the following details:
